GRAHAM'S GIFT CHILDREN'S FOUNDATION CONTRACT                       


Graham’s Gift Children’s Foundation (GGCF) hereby agrees to the following:

1.        To provide home study services to the Prospective Adoptive Parents ( Parents) for the total fee of  $1,100 with
$600 payable directly to their Social Worker  ( $700 for China) and $500 payable separately to GGCF or as agreed
upon depending on location and any additional requirements of placement agency. ( total $1200- or $1300 for China)
2.        To honestly communicate with  Adoptive Parents with respect and consideration.
3.        To provide the  Parents with a complete package of  directions, clearance forms, fingerprint cards, envelopes
and all form templates necessary to complete the adoption
4.        To provide an educational C/D of adoptive parent training consisting of over 10 hours of reading materials
5.        To process all NYS legal clearances in a timely manner upon receipt
6.        To be available for guidance and questions during business hours throughout the adoption process and after
placement
7.        To promptly return emails and phone calls to the Parents usually within 24 hours
8.        To assign a Master’s level, adoption experienced,  Social Worker to their case who will interview them, discuss
the educational materials and visit their home  and write the home study , once all paperwork  and clearances have  
been received
9.        To evaluate all submitted materials in a fair and professional manner
10.        To write the home study in a manner that conforms to the placement agency’s guidelines
11.        To write the home study to comply with the sending country, state or other guidelines provided to GGCF
12.        To provide signed interagency agreements  with Parent’s placement agency where, in the opinion of the
Director, they are reasonable, legally appropriate and in the best interests of parents and children
13.        To provide any other  properly requested materials or documentation  to other adoption providers
14.        To provide the Parents and/or their placement agency with a copy of all licenses and authorizations for GGCF
and its employees and/or agents
15.        To complete the home study in a timely manner and at no time  later than  14 days  from the time all clearances,
paperwork and interviews are completed and in our possession
16.        To keep all Parent information confidential except for materials  consented to be released to adoption related
agencies, governmental  authorities or others for adoption purposes
17.        To review referral information with  adoptive parents when provided  with it and assist them in making decisions
concerning it
18.        To review the Parent’s report of adoption with decree and child’s medical form
19.        To be available post placement/adoption to advise, counsel, refer and assist in adjustment of the child
20.        To perform Post Placement and/or Post Adoption  reports according to the sending country, state, placement
agency, courts, ICPC or other standards upon receipt of  timelines, guidelines and request at a rate of $250 per visit
and report ($50 additional for more than one child)
21.        To  provide services to  the Parents and  attempt to find an alternative placement should  Parents desire to
disrupt or dissolve the placement at additional cost to Parents,  if necessary.  
22.        NOTE: GGCF does not provide temporary custody or respite services.  It does not accept legal responsibility for
re-placement of  children whose adoptions dissolve or disrupt
23.        To provide future updates to the home study as needed at a discounted rate of $200 less than the original
depending upon circumstances and subject to change
24.        To provide quality, ethical, respectful, timely professional  services and responsive practices
25.        That Parents have the right to complain of inadequate, improper  or  unsatisfactory services to GGCF and to
authorities
26.        To listen to Parent concerns and complaints and do all it can to remedy them

27.        That GGCF and its employees and/or agents will NOT retaliate  in any way following complaint
28.        That  listed herein  are a list of authorities to whom complaints may be addressed if GGCF does not resolve
complaints
29.        To represent  and advise the Parents interests primarily as its clients and  objectively and without reference to
business consideration or other agency’s interests  so long as it is consistent with the best interests of children.


The Prospective Adoptive Parent(s) hereby agree(s) to the following:

1.        To pay for an adoptive home study for the total fee of  $1,200  ( $1,300 for China) with $600 payable directly to
their Social Worker  ( $700 for China) and $600 payable separately to GGCF or as agreed upon  depending upon
location or additional requirements.  Mileage fees will be assessed if travel is more than 30 miles from SW.
2.        That payment will be made as follows:
$50 non refundable application fee with intake form
$550 nonrefundable agency fee  upon submission of clearances
$600 ($700 China) to Social Worker at first home visit
3.        To be truthful and accurate in providing information concerning their application to adopt
4.        To call  or email GGCF AGAIN in the event that communication is not returned within 24 hours by GGCF or one
of its Social Workers
5.        To provide all required   documentation  to GGCF and the Social Worker  in a timely manner
6.        To read and discuss all provided parent educational material which is required
7.        To ask any questions they may have as a result of the adoption education
8.        To provide any placement agency or sending state or country requirements  (if known) for the home study to
GGCF BEFORE  meeting with Social Worker
9.        That they understand there may be additional charges for additional home visits or additional documentation
required by a placement agency, state or country
10.        To have three people who know the applicants well  and are able to be verified  write an honest  reference letter
and  have them send it to both the Social worker and copy to GGCF
11.        To submit the provided home study approval form to GGCF upon receipt of approved home study draft.
12.        That  they understand that  completed home study will not be issued until all documentation, clearances,
payment for services and home study approval form are received
13.        That they acknowledge that GGCF has no control  and is not responsible  for  the timing of NYS and Federal
clearances and that the home study may be delayed if prints are rejected, clearance forms are sent back for editing or
other corrections.  This will result in delays of issuing home study
14.        That they acknowledge that no home study fees are refundable  if the applicant is denied approval or changes
its mind about adopting
15.        That the home study may be discontinued by mutual consent with no refund of paid fees except  (partial fees
may be returned  under exceptional circumstances at the discretion of the Director)
16.        To submit to any additional evaluations at the Parents  expense that are deemed necessary by the Agency to
ascertain the suitability of the applicant(s) as adoptive parents
17.        To inform GGCF as soon as they become aware of the post placement or post adoption monitoring
requirements of the Country, State or placement agency
18.        That they will not knowingly cooperate in or agree to the adoption or placement  of any child they believe to
have been bought, sold, kidnapped or otherwise  trafficked as the result of corruption or illegal activity.
19.        That they will  advise GGCF of any unsatisfactory services and give GGCF the opportunity to correct it

20.        That they  acknowledge that they have been informed of the following rights:
a.)        The right to lodge a complaint at any time with this agency
b.)        The right to lodge a complaint with authorities and have been advised below  where  to do so
c.)        In the event you are denied approval of your home study  you will be issued a written rejection and are entitled
to a hearing pursuant to Sec 22 and 372e of the NYS  Social Services Law.
21.        That they will notify GGCF upon the completion of their adoption and provide within 10 days of returning home a
copy of the child’s adoption decree or order of placement and GGCF Report of Completed Adoption with a copy of the
child’s medical form.  They will call their assigned Social Worker to discuss adjustment within 7 days of arrival at home.
22.        That they will submit to post placement monitoring at a fee of $250 per evaluation and report  and report and do
so at 6 months and one year and as specified by the placement agency or   country.   There may be a travel surcharge
for some locations.  Parents agree to submit to additional post placement monitoring at their expense if in the judgement
of GGCF’s Social Worker and Director it is needed.
23.        That they will obtain any additional recommended evaluation, counseling or treatment at their expense  that
GGCF’s Director and Social Worker  recommends.
24.        That they will contact GGCF in the event that they have any  adjustment difficulties out of the ordinary, medical
concerns about the child  or are contemplating possible disruption or dissolution of the adoption
25.        That they will not dissolve or disrupt without informing GGCF of this regardless of when this occurs
26.        That they will agree to additional counseling at their own  expense  before dissolving or disrupting an adoption
27.        That they understand that GGCF does not provide respite or  temporary custody  pending re-placement of a
disrupted or dissolved adoption
28.        That they understand that GGCF is not legally responsible for the re-placement of a child
29.        That they understand that this agreement, in particular with reference to post placement  or adoption  reports, is
a binding contract.  They will be held liable for all costs and legal fees associated with the enforcement of the post
placement provisions of this contract.
30.        That they will not use corporal punishment as a discipline method.

submitted)  fee to Graham’s Gift Children’s Foundation.   Upon receipt we will send out our full home study packet.

A decision concerning this application will be made after  receipt of all required information  from applicant(s)  and
government entities within 10 days.  You will be issued a written report which you should read and approve of by signing
and returning the form indicating you have done so.  In the event you are denied approval  you will be issued a written
rejection and are entitled to a hearing pursuant to Sec 22 and 372e of the NYS  Social Services Law.
